Частота неудовлетворительных отдаленных результатов лечения варикозной болезни нижних конечностей (ВБНК) на протяжении последнего десятилетия не имеет тенденции к снижению, поэтому проблема рецидива заболевания по-прежнему не теряет своей злободневности. Цель исследования. Оптимизация конечных результатов лечения рецидивов варикозной болезни нижних конечностей за счет внедрения методики тотальной эндовазальной лазерной коагуляции («total-ЭВЛК»). Материал и методы исследования. В работе проанализированы результаты диагностики и лечения 17 пациентов с истинными рецидивами ВБНК. Все лица были оперированы повторно в УКБ № 4 Сеченовского Университета за период с 2021 по 2024 гг. с применением методики «total-ЭВЛК». Результаты. Технический и клинический успех повторной хирургической коррекции рецидива ВБНК достигнут во всех наблюдениях, осложнений не отмечено. Заключение. Обоснованное применение ЭВЛК у лиц с рецидивом ВБНК позволяет избежать травматичных манипуляций в области рубцово-измененных тканей, существенно снизить риск повреждения важных анатомических структур, уменьшить продолжительность операции, улучшить качество жизни пациентов, повысив при этом конечный эстетический результат лечения. Технологию «total-ЭВЛК» можно рекомендовать для радикальной коррекции рецидивов ВБНК – независимо от локализации и объема поражения подкожной венозной системы.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. The problem of recurrence of varicose veins of the lower extremities remains relevant. The purpose of the study. The aim of our study was to optimize the results of varicose veins recurrence treatment by introducing the total endovasal laser ablalation (total-EVLA) technique. Material and methods. The work analyzes the results of diagnostics and treatment of 17 patients with true recurrences of varicose veins. All individuals were re-operated at the Clinical Hospital № 4 of Sechenov University at 2021–2024 period using the "total-EVLA" technique. Results. Technical and clinical success of repeated surgical correction of varicose veins recurrence was achieved in all cases. Conclusion. Use of EVLA among patients with recurrent varicose veins avoids traumatic manipulations in the area of cicatrical-changed tissues in order to significantly reduce the risk of damage to important anatomical structures, reduce the duration of the surgery, improve patients’ quality of life of, while increasing the final aesthetic result of the treatment.
