Протезирование торакоабдоминального отдела аорты (ТАА) является сложным хирургическим вмешательством, требующим реконструкции и реимплантации рено-висцеральных ветвей (РВВ). Выбор оптимального метода реимплантации РВВ играет важную роль в достижении долгосрочного успеха операции.</p></sec><sec><title>Цель</title><p>Цель. Проанализировать и оценить предоперационные параметры МСКТ-аортографии и анатомо-морфологические характеристики РВВ и определить их влияние на выбор метода реимплантации РВВ при реконструкции ТАА.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В РНЦХ им. Б.В. Петровского в рамках исследования проведен ретроспективный анализ данных у 120 пациентов с 01.2018 по 09.2023 гг., которым было выполнено протезирование ТАА с имплантацией всех РВВ. Оценены предоперационные данные и параметры аорты и РВВ по данным МСКТ-аортографии. Пациенты разделены на три группы; группа I — «Crawford» (n=43, 35,8 %), гр. II — «Coselli» (n=32, 26,7 %) и III гр. — «кнопки» (n=45, 37,5 %).</p></sec><sec><title>Результаты</title><p>Результаты. 1) «Crawford» следует выбирать у пожилых пациентов, с близким расположением устьев РВВ по отношению друг к другу, при отсутствии ДСТ и дегенеративного поражения РВВ. 2) «Coselli» целесообразно у пациентов с большим диаметром аневризмы на уровне РВВ (p&lt;0,001). Методика имеет преимущество у больных с «shaggy aorta» и морфологически измененными РВВ (стеноз, окклюзия, расслоение) — р=0,001 и р&lt;0,050, соответственно. 3) Метод «кнопки» целесообразно применять при удаленном рено-висцеральном расположении (р=0,001), у больных с ДСТ (р=0,012) и при вариантной анатомии РВВ (р&lt;0,050).</p></sec><sec><title>Заключение</title><p>Заключение. Предоперационное планирование на основе антропометрических данных и МСКТ позволяет разработать стратегию реимплантации РВВ, учитывая патологию аорты и минимизируя риск осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Thoracoabdominal aortic (TAA) replacement is a complex surgical procedure that requires reconstruction and reimplantation of the renovisceral branches (RVB). The choice of the optimal method for RVB reimplantation plays an important role in achieving long-term success of the operation.</p></sec><sec><title>Objective</title><p>Objective. To analyze and evaluate preoperative parameters of CTA aortography and anatomical and morphological characteristics of RVB and determine their influence on the choice of RVB reimplantation technique during TAA reconstruction.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. At the B.V. Petrovsky Russian Scientific Center of Surgery, a retrospective analysis of data from 120 patients was carried out as part of the study from 01.2018 to 09.2023, who underwent TAA prosthetics with implantation of all RVB. Preoperative data and parameters of the aorta and RVB according to MSCT aortography were evaluated. Patients were divided into three groups; group I — «Crawford» (n=43, 35,8 %), gr. II — «Coselli» (n=32, 26,7 %), III gr. — «buttons» (n=45, 37,5 %).</p></sec><sec><title>Results</title><p>Results. 1) «Crawford» should be chosen in elderly patients with close location of the RVB ostia in relation to each other, in the absence of connective tissue disease (CTD) and degenerative lesions of the RVB. 2) «Coselli» is appropriate for patients with a large diameter of the aneurysm at the level of the RVB (p&lt;0,001). The technique has an advantage in patients with "shaggy aorta" and morphologically altered RVB (stenosis, occlusion, dissection) – p=0,001 and p&lt;0,050, respectively. 3) The «button» method is advisable to use with a remote reno-visceral location (p=0,001), in patients with CTD (p=0,012) and with variant RVB anatomy (p&lt;0,050).</p></sec><sec><title>Conclusion</title><p>Conclusion.
