В статье представлен анализ бактериологических исследований желчи и выпота брюшной полости у 32 больных с острым билиарным панкреатитом в раннем послеоперационном периоде. Острый билиарный панкреатит составляет до 60 % всех случаев острого воспаления поджелудочной железы. Инфицирование при деструктивных формах достигает 70 %, значительно повышая летальность. Современная стратегия антибактериальной терапии требует бактериологического подтверждения контаминации.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить риск развития бактериальной контаминации желчи и выпота брюшной полости при остром билиарном панкреатите в раннем послеоперационном периоде.</p><p>Материалы и методы исследования. Проведено исследование проб желчи и выпота у 32 пациентов после лапароскопической холецистэктомии с дренированием. Бактериологические исследования выполнялись по стандартным методикам с оценкой качественных и количественных показателей. Забор проб осуществлялся в первые сутки после операции и через 3 дня.</p></sec><sec><title>Результаты</title><p>Результаты. Риск контаминации желчи и выпота был высоким (43,75±8,77 % и 37,50±8,56 % соответственно). К третьим суткам отмечено снижение риска контаминации до 28,13±7,95 % и 25,00±7,65 %. У больных с тяжелым течением заболевания сохранялся значимо высокий уровень контаминации обеих сред. Преобладающим возбудителем являлась Escherichia coli, преимущественно в монокультуре. Уровень бактериальной обсемененности в большинстве случаев не превышал 104 КОЕ/г.</p></sec><sec><title>Заключение</title><p>Заключение. Наиболее частым возбудителем является Escherichia coli. Наименьший уровень антибиотикорезистентности отмечен к цефалоспоринам IV поколения, карбапенемам, ванкомицину.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The article presents an analysis of bacteriological studies of bile and abdominal effusion in 32 patients with acute biliary pancreatitis in the early postoperative period. Acute biliary pancreatitis accounts for up to 60 % of all cases of acute inflammation of the pancreas. Infection with destructive forms reaches 70 %, significantly increasing mortality. The current strategy of antibacterial therapy requires bacteriological confirmation of contamination.</p></sec><sec><title>The purpose of the study</title><p>The purpose of the study. Bile and effusion samples were examined in 32 patients after laparoscopic cholecystectomy with drainage. Bacteriological studies were performed according to standard methods with an assessment of qualitative and quantitative indicators. Sampling was carried out on the first day after the operation and 3 days later.</p></sec><sec><title>Results</title><p>Results. The risk of bile contamination and effusion was high (43,75±8,77 % and 37,50±8,56 %, respectively). By the third day, there was a decrease in the risk of contamination to 28,13±7,95 % and 25,00±7,65 %. In patients with severe disease, a significantly high level of contamination of both media was maintained. The predominant pathogen was Escherichia coli, mainly in monoculture. The level of bacterial.</p></sec><sec><title>Conclusion</title><p>Conclusion. The most common pathogen is Escherichia coli.
