На долю хирургических пациентов приходится почти треть всех случаев сепсиса, лечение которого характеризуется высокими затратами, длительностью, риском осложнений. Биомаркеры при ранней диагностике позволяют могут сыграть решающую роль в оценке эффективности проводимого лечения и дифференциальной диагностики.</p></sec><sec><title>Цель</title><p>Цель. Изучить особенности диагностики сепсиса с помощью биомаркеров, эффективность их применения в хирургической практике.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Выполнен обзор данных в PubMed, CochraneLibrary, ScienceDirect, eLIBRARY. Были применены следующие критерии для исключения анализируемых материалов: описание отдельных клинических случаев; книги и документы; сравнение результатов лечения. В проведенный анализ из 116 первично выявленного источника включены 30.</p></sec><sec><title>Результаты</title><p>Результаты. На сегодняшний день выделяют несколько групп биомаркеров, применяемых при данной патологии. Постоянно повышенные уровни CRP и PCT коррелируют с септическими осложнениями. Но для надёжной интерпретации результатов их следует определять в динамике. Из группы провоспалительных цитокинов наиболее часто применяется IL-6, но он недостаточно надёжен как критерий, ограниченно используется в клинической практике. Пресепсин лучше всего подходит для ранней диагностики, но его использование ограничивается высокой стоимостью исследований и отсутствием четко установленного порога.</p></sec><sec><title>Заключение</title><p>Заключение. Биомаркеры широко применяются в ранней диагностике сепсиса, прогнозировании исхода и как дополнительный критерий при выборе рациональной антибиотикотерапии. Однако, до сих пор «идеальный» биомаркер сепсиса не определен, требуются глубокая объективная интерпретация их использования, а также оценка возможности их комбинированного применения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Surgical patients account for almost a third of all cases of sepsis, its treatment means higher costs, longer duration, risk of complications. Biomarkers in case of early diagnosis and can also play a crucial role in assessing the effectiveness of treatment.</p></sec><sec><title>The purpose of the study</title><p>The purpose of the study. To study the features of sepsis diagnostics using biomarkers and their effectiveness in surgical practice.</p><p>Materials and methods of research. Data were reviewed in PubMed, CochraneLibrary, ScienceDirect, eLIBRARY. The following criteria were applied to exclude the analyzed materials: description of individual clinical cases; books and documents; comparison of treatment results. 116 sources were initially identified and 51 sources were included in the analysis.</p></sec><sec><title>Results</title><p>Results. Today there are several groups of biomarkers used in this pathology. Consistently elevated CRP and PCT levels correlate with septic complications. But for a reliable interpretation of the results, they should be determined over time. In the group of proinflammatory cytokines, IL-6 is the most commonly used but it is not reliable enough as a criterion and is used to a limited extent in clinical practice. Presepsin is best suited for early diagnosis but its use is limited by high cost of tests and lack of a clearly defined threshold.</p></sec><sec><title>Conclusion</title><p>Conclusion. Biomarkers are widely used in the early diagnosis of sepsis, predicting the outcome and as an additional criterion for choosing rational antibiotic therapy.
