Степень лимфогенного распространения рака ободочной кишки определяет тактику лечения и прогноз для пациента.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Изучить характер метастазирования в лимфоузлы рака левой половины ободочной кишки.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. За период с марта 2022 по декабрь 2023 было проанализированы 53 патоморфологических исследования операционного материала после предварительного детального препарирования хирургом.</p></sec><sec><title>Результаты лечения</title><p>Результаты лечения. После сегментарной резекции кишки с D3-лимфодиссекцией и селективным пересечением сосудов было удалено в среднем 62 (IQR: 38-20) регионарных лимфоузла. При D3-лимфодиссекции, выполняемой со скелетизацией ствола НБА, было удалено в среднем 11 (IQR: 6-2) апикальных лимфоузлов. У пациентов с 3 стадией было удалено в среднем 13 (IQR: 9-1) метастатических лимфоузлов. В параколической группе в среднем выявлено 9 метастатических лимфоузлов (IQR: 4-0), в промежуточной группе – 4 лимфоузла (min-max 0–8), а в апикальной группе – до 1 метастатического лимфоузла. Метастазирование в апикальную группу лимфоузлов наблюдалось 5,7 % случаев в общей группе исследованных пациентов. Метастазы в апикальные лимфоузлы выявлены у пациентов с раком сигмовидной кишки и составили 7,1 % наблюдений.</p></sec><sec><title>Заключение</title><p>Заключение. Поражение апикальной группы лимфоузлов метастазами наблюдается у пациентов с раком сигмовидной кишки. Полная лимфодиссекция с опухоль-специфичной мезоколонэктомией позволяет добиться более точного стадирования и радикального лечения у пациентов с левосторонней локализацией опухоли.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The extent of lymphatic spread of cancer in colon lesions determines the treatment options and prognosis for the patient.</p></sec><sec><title>The purpose of the study</title><p>The purpose of the study. To study the pattern of lymph node metastasis in left colon cancer.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 53 pathomorphological studies of the surgical material after preliminary detailed preparation by the surgeon were analyzed from March 2022 till December 2023.</p></sec><sec><title>Treatment results</title><p>Treatment results. After segmental colonic resection with D3 lymph node dissection and selective vascular ligation, a median number of 62 (IQR: 38-20) regional lymph nodes were excised. A median of 11 (IQR: 6-2) apical lymph nodes were removed. The median number of metastatic paracolic nodes was 9 lymph nodes (IQR: 4-0). There were 4 metastatic intermediate lymph nodes in median (min-max: 0–8), and 1 metastatic lymph node in the apical group. The metastatic spread in apical lymph nodes occurred in 3 of 53 patients (5,7 %,). Apical lymph node metastasis occurred only in sigmoid colon cancer in 7,1 % of cases.</p></sec><sec><title>Conclusion</title><p>Conclusion. Metastatic apical lymph node spread is found in sigmoid colon cancer.
