В доступной литературе отсутствует информация о преимуществах или недостатках использования разных систем безузловой фиксации тканей при лапароскопической гастроэнтеростомии, что затрудняет их обоснованный выбор. Цель исследования. Выполнить сравнительную оценку непосредственных результатов применения самофиксирующихся мононитей со спиральной и оппозиционной ориентацией насечек при формировании гастроеюноанастомоза в лапароскопическом минигастрошунтировании (ЛМГШ).Материалы и методы. У 80 пациентов, включенных в проспективное исследование, на этапе мануального формирования гастроеюноанастомоза ЛМГШ использовались системы безузловой фиксации тканей: в первой группе (38 пациентов) – со спиральной ориентацией однонаправленных насечек, во второй (42 пациента) – с оппозиционной. Группы были сопоставимы по демографическим характеристикам, индексу массы тела, характеру коморбидной патологии и частоте ранее перенесенных операций.Результаты. Продолжительность интраабдоминального этапа ЛМГШ, а также время наложения гастроеюноанастомоза в выделенных группах значимо не отличались. Гастроеюностомия с помощью систем безузловой фиксации тканей, в среднем, занимала до 24,4 % операционного времени. Интраоперационные осложнения первой степени тяжести по системе Satava-Kazaryan зарегистрированы с частотой 5,3–7,1 % (р=0,999). В послеоперационном периоде развитие малых осложнений (первой степени тяжести системы Clavien-Dindo-Strasberg) имело место у 7,9 % и 11,9 % пациентов первой и второй групп, соответственно (р=0,715).Заключение. Непосредственные результаты использования самофиксирующихся нитей с оппозиционным и спиральным расположением однонаправленных насечек при мануальном формировании гастроэнтероанастомоза в ЛМГШ сопоставимы.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. The available literature lacks information on the advantages of different knotless tissue fixation devices for the laparoscopic intracorporeal gastroenterostomy, making their informed choice difficult.The purpose of the study. To evaluate the short-term clinical outcomes of the monofilament barbed sutures with a helical and an oppositional barbes design in the single anastomosis formation during laparoscopic mini-gastric bypass (LMGB).Material and methods. Knotless tissue fixation devices were used for manual intracorporeal formation of gastrojejunal anastomosis during LMGB in 80 patients: with a helical barbes design – in the first group (38 patients) and with an oppositional – in the second group (42 patients). No significant differences were detected in the baseline demographic characteristics, body mass index, comorbidity and the frequency of previous abdominal surgeries between groups.Results. There were no differences in total intraabdominal operation time and the duration of the gastrojejunal anastomosis formation in the clinical groups. Gastrojejunostomy using knotless tissue fixation devices, on average, took up to 24,4% of the operation time. Intraoperatively, only incidents of severity grade 1 (Satava-Kazaryan classification) were recorded with a frequency of 5,3–7,1 % (р=0,999). Postoperatively, minor complications (severity grade 1 of the Clavien-Dindo-Strasberg system) were recorded in 7,9 % and 11,9 % of patients in the first and second clinical groups, respectively (p=0,715).Conclusion.
