Провести анализ комбинированного способа хирургического лечения внутреннего геморроя методом подслизистой лазерной деструкции в сочетании с допплер-неконтролируемой дезартеризацией, мукопексией и электроэксцизией наружного компонента по показаниям в сравнении с традиционными электрохирургическими методиками.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 1386 пациентов с геморроем 2, 3 и 4 стадии, которые были распределены на три группы в зависимости от проведенного метода хирургического лечения. Пациентам первой группы выполнялась геморроидэктомия по методике Миллигана-Моргана в модификации НИИ проктологии 2 с применением электрокоагулятора под спинальной анестезией.</p><p>Пациентам второй группы, выполнялось аналогичное хирургическое лечение, но с применением аппарата для биполярной электрорезекции также под спинальной анестезией. В третьей группе всем пациентам выполнялась субмукозная лазерная деструкция внутренних геморроидальных узлов с допплер-неконтролируемой дезартеризацией, дополненной по показаниям мукопексией и электроэксцизией наружного геморроидального компонента. Хирургическое лечение выполнялось под внутривенным обезболиванием. Применялось лазерное излучение диодного аппарата с длиной волны 1,56 мкм мощность – 10 Вт.</p></sec><sec><title>Результаты</title><p>Результаты. Гибридная методика снижает риск послеоперационного кровотечения за счет лучшего гемостаза в ближайшем послеоперационном периоде и раннего формирования фиброзно-грануляционной ткани в позднем послеоперационном периоде. Позволяет избежать формирования грубой рубцовой ткани.</p></sec><sec><title>Заключение</title><p>Заключение. Методика обладает технической простотой, возможностями индивидуального подхода к пациенту, не имеет ограничений по объему и стадии заболевания. Позволяет снизить уровень послеоперационных осложнений, ускорить сроки реабилитации, повысить качество жизни пациентов. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objectives</title><p>Objectives.To analyze the combined method of surgical treatment of internal hemorrhoids by submucosal laser destruction in combination with Doppler uncontrolled desarterization, mucopexy and electroexcision of the external component according to indications in comparison with traditional electrosurgical techniques.</p><p>Material and methods of research. The study included 1386 patients with hemorrhoids of stages 2, 3 and 4, who were divided into three groups depending on the method of surgical treatment. Patients of the first group underwent hemorrhoidectomy according to the Milligan-Morgan method in the 2 modification of the State Proctology Research Institute with the use of an electrocoagulator under spinal anesthesia.</p><p>Patients of the second group underwent similar surgical treatment, but with the use of a bipolar electrosection device also under spinal anesthesia. In the third group, all patients underwent submucous laser destruction of internal hemorrhoids with Doppler uncontrolled desarterization, supplemented according to indications with mucopexy and electroexcision of the external hemorrhoidal component. Surgical treatment was performed under intravenous anesthesia. Laser radiation of a diode device with a wavelength of 1,56 microns and a power of 10 W was used.</p></sec><sec><title>Treatment results</title><p>Treatment results.The hybrid technique reduces the risk of postoperative bleeding due to better hemostasis in the immediate postoperative period and early formation of fibrous granulation tissue in the late postoperative period. Avoids the formation of rough scar tissue.</p></sec><sec><title>Conclusion</title><p>Conclusion. The technique has technical simplicity, the possibilities of an individual approach to the patient, has no restrictions on the volume and stage of the disease.
