Количество исследований, сравнивающих уровни бактериальной контаминации билиарного дерева и чувствительность полученных бактерий к антибиотикам в посевах желчи, полученной при первичных и повторных эндоскопических ретроградных холангиографиях (ЭРХГ), крайне ограничено, однако изучение данного вопроса может значительно повлиять на тактику лечения механической желтухи.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Сравнение микробиологической картины желчи и чувствительности основных возбудителей к антибактериальной терапии при первичных и повторных эндобилиарных вмешательствах с определением клинической значимости метода.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 43 пациента с ЭРХГ при стриктурах терминального отдела холедоха (20 пациентов с наличием ЭРХГ в анамнезе, 23 пациента с впервые выполняемой ЭРХГ). Сравнивались титры выявленных возбудителей, чувствительность основных возбудителей к антибиотикотерапии.</p></sec><sec><title>Результаты</title><p>Результаты. Всего положительных посевов выявлено 86,96 % при первичных вмешательствах и 100 % при повторных. Наиболее часто встречающиеся бактерии – Esherichia coli (E. coli) и Klebsiella pneumoniae (K. pneumoniae). Облигатные анаэробы ни в одном случае выявлены не были. E. coli и K. pneumoniae продемонстрировали полирезистентость к большей части антибактериальных препаратов.</p></sec><sec><title>Заключение</title><p>Заключение. Внедрение забора желчи при выполнении эндобилиарных вмешательств для проведения микробиологического анализа с определением чувствительности к антибиотикам в клиническую практику может значительно повлиять на выбор эмпирической и этиотропной антибиотикотерапии и улучшить результаты лечения. Полирезистентность бактерий обусловливает важность проведения аналогичных исследований в других стационарах с целью рационального назначения антибактериальных препаратов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Te number of studies comparing the levels of bacterial contamination of the biliary tree and the sensitivity of the resulting bacteria to antibiotics in cultures of bile obtained from primary and repeated endoscopic retrograde cholangiography (ERCG) is extremely limited, but the study of this issue can signifcantly aﬀect the tactics of treating obstructive jaundice.</p></sec><sec><title>Te purpose of the study</title><p>Te purpose of the study. Compare the microbiological picture of bile and the sensitivity of the main pathogens to antibiotic therapy in primary and repeated endobiliary interventions with the determination of the clinical signifcance of the method.</p><p>Study material and methods. Te study included 43 patients with ERCG with  strictures of the terminal choledochus (20 patients with a history of ERCG, 23 patients with frst-time ERCG). Te titers of the identifed pathogens and the sensitivity of the main pathogens to antibiotic therapy were compared.</p></sec><sec><title>Outcomes</title><p>Outcomes. 86,96 % of positive cultures were detected during primary interventions and 100 % during repeated ones. Te most common bacteria are Escherichia coli (E. coli) and Klebsiella pneumoniae (K. pneumoniae). Obligate anaerobes were not detected in any of the cases. E. coli and K. pneumoniae have shown polyresistance to most antibacterial drugs.</p></sec><sec><title>Conclusion</title><p>Conclusion. Te introduction of bile sampling during endobiliary interventions for microbiological analysis with the determination of sensitivity to antibiotics in clinical practice can signifcantly aﬀect the choice of empirical and etiotropic antibiotic therapy and improve treatment outcomes.
