Цель: изучить результаты применения смешанной реальности в ходе лапароскопической резекции почки.Материалы и методы. В проспективное исследование были включены 47 пациентов, которым была выполнена лапароскопическая резекция почки с июня 2020 года по февраль 2021 года. Пациенты были разделены на две группы случайным образом: контрольная (24 пациента), в которой использовалась интраоперационная ультразвуковая навигация (УЗИ), и группа исследования (23 пациента), в которой операция выполнялась с использованием дополненной реальности: в смарт-очках была воссоздана индивидуальная объемная модель органа, пораженного опухолью. Модель была создана в программном обеспечении HLOIA©, разработанном нашей командой. По каждому пациенту были собраны анатомические, демографические данные, результаты инструментального обследования в предоперационном периоде, периоперационные данные. Оценка применения дополненной реальности производилась с использованием анкет по 5-ти балльной шкале Лайкерта, заполняемых хирургом после операции.Результаты. Сравнение данных выявило статистически значимые различия по следующим параметрам: время поиска и визуализации почечной ножки, время выявления локализации опухоли (р &lt;0,001), которые были в пользу группы исследования. Впечатление хирурга о полезности модели в смешанной реальности (MR), по результатам анкетирования, продемонстрировало высокую оценку во всех утверждениях.Выводы. Технология смешанной реальности стала доступна с использованием смарт-очков и программного обеспечения HLOIA©. Она показала уменьшение времени, необходимом для диссекции и выхода на почечную ножку, и времени необходимым для визуализации границ и локализации опухоли почки.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Purpose: to advance the laparoscopic partial nephrectomy (LPN) through the use of mixed reality (MR) technology.Materials and methods. 47 patients with LPN were enrolled in the prospective research from June 2020 to February 2021 after institutional review board approval. Patients were randomly assigned to two different groups: the control group, which included 24 patients, was operated with an intraoperative ultrasound (US) control, while the experimental group, which included 23 patients, was operated with smart glasses with the individual MR model of the organ with a tumor. Our team takes credit for the creation of an open-source software package, “HLOIA,” which was tested in the experimental group. Demographic, perioperative, and pathological data sets were collected separately for each individual patient. A 5-point Likert scale questionnaire was used to evaluate the utility of an MR model during LPN. It was completed by the surgeon immediately after LPN.Results. Experimental group outperformed the control group in several criteria, such as the time for renal pedicle exposure and the time from the renal pedicle to the detection of tumor localization (p &lt;0.001). The questionnaire, filled out by the surgeon, revealed high utility scores of the MR model.Conclusions.
