Цель исследования: изучение механизмов возникновения и особенностей диагностики спонтанных внутримышечных кровоизлияний у пациентов с вирусной пневмонией COVID-19.</p></sec><sec><title>Методы исследования</title><p>Методы исследования. Проанализирован анамнез, клинико-лабораторные данные и результаты компьютерной томографии 28 пациентов со спонтанными внутримышечными гематомами на фоне вирусной пневмонии. Результаты. По данным компьютерной томографии диагностированы 36 спонтанных гематом мышц и мягких тканей различной локализации (у 7 пациентов множественные гематомы), преимущественно у женщин, средний возраст пациента 75 лет. Артериальные источники кровотечения по данным компьютерной томографии с болюсным контрастированием не обнаружены. Корреляция объема и сроков развития гематом с антикоагулянтной терапией не выявлена. Отмечено развитие 57,1 % гематом с 21-х суток от начала заболевания. В динамике объем гематом не нарастал и не требовал хирургического вмешательства.</p></sec><sec><title>Заключение</title><p>Заключение. Спонтанные внутримышечные кровоизлияния – редкое осложнение вирусной пневмонии COVID-19, приводящее к неблагоприятному исходу только при большом объеме гематомы. Хирургическое лечение гематом не требуется. На развитие спонтанных гематом могут оказывать влияние как назначенная антикоагулянтная терапия, так и непосредственное воздействие вируса SARS-CoV-2 на свертывающую систему крови и эндотелий стенки мелких сосудов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Purpose. Study the mechanisms of occurrence and the imaging characteristics of spontaneous intramuscular hemorrhage in patients with COVID-19 viral pneumonia.</p></sec><sec><title>Methods</title><p>Methods. The anamnesis, clinical and laboratory data and the results of computed tomography of 28 patients with spontaneous intramuscular hematomas associated with viral pneumonia were analyzed.</p></sec><sec><title>Results</title><p>Results. According to computed tomography, 36 spontaneous hematomas of muscles and soft tissues of various localization were diagnosed (7 patients had multiple hematomas), mainly in women, the average age of the patient was 75 years. No arterial sources of bleeding were detected by contrast enhanced computed tomography. There was no correlation between the volume and timing of the development of hematomas with anticoagulant therapy. The appearance of 57.1% of hematomas after the 21st day from the onset of the disease was noted. In dynamics, the volume of hematomas didn’t increase and did not require surgical intervention.</p></sec><sec><title>Conclusion</title><p>Conclusion. Spontaneous intramuscular hemorrhage is a rare complication of COVID-19 viral pneumonia, leading to an unfavorable outcome only with a large volume of hematoma. Surgical treatment of hematomas is not required.
