Значимое и стойкое снижение веса у женщин с раком тела матки и морбидным ожирением благоприятно влияет на онкологические результаты, дальнейшее качество жизни, течение сопутствующей патологии. Лапароскопическая гистерэктомия является приоритетным вариантом лечения рака тела матки ранних стадий. Бариатрическая хирургия в настоящий момент считается наиболее эффективным и радикальным методом коррекции тяжелых форм ожирения.Клинический случай: случаи выполнения бариатрических операций с целью позитивного влияния на результаты лечения злокачественных новообразований до или после основного этапа в последнее время участились. Опыт одноэтапного проведения симультанной радикальной операции по поводу злокачественного новообразования и коррекции морбидного ожирения в мировой практике пока единичный. Представляем клинический случай одномоментного выполнения лапароскопической рукавной резекции желудка и лапароскопической гистерэктомии у женщины 45 лет по поводу злокачественного новообразования эндометрия IА стадии с морбидным ожирением и сахарным диабетом II типа. Ближайшими результатами данного наблюдения является потеря 21,2% от избыточной массы тела через 2 месяца, стойкая ремиссия сахарного диабета, нахождение в III клинической группе.Обсуждение: описанный случай теоретически обоснован и имеет, хотя и не многочисленные, практические прототипы, которые появляются в публикациях последних лет после успешного клинического применения.Заключение: имеющийся пока что небольшой мировой опыт подобных случаев требует дальнейшего накопления и отработки практических аспектов и более детального изучения данной тематики.</p></abstract><trans-abstract xml:lang="en"><p>Introduction: endometrial cancer has a direct correlation with obesity. Risks of this type of cancer increase with the body weight gain and expected response to treatment becomes less favorable. Significant and persistent weight loss of hysterocarcinoma female with morbid obesity has a positive impact on oncological results, further quality of life, state of comorbidity. Laparoscopic hysterectomy is a priority treatment of early endometrial cancer. Bariatric surgery is considered to be the most effective and radical method of morbid obesity correction.Clinical case: the cases of bariatric surgery to influence positively the results of carcinoma treatment outcome before and after the main stage have increased recently. An experience of concurrent simultaneous radical surgery of сancer and morbid obesity correction is isolated worldwide nowadays. We would like to present the clinical case of concurrent laparoscopic sleeve gastrectomy and laparoscopic hysterectomy of a 45-year-old female patient with endometrial carcinoma stage IA, morbid obesity and 2 type diabetes mellitus.
