<trans-abstract xml:lang="en"><p>Introduction. Routine open arterial reconstructions in the infrainguinal segment are accompanied by early thrombosis of the grafts (up to 5 % of cases), which requires repeat surgical interventions. According to the literature, failure to use intraoperative imaging leads to residual defects in the arterial bed of the lower limbs, a higher incidence of thrombosis, and repeat interventions (up to 36 % within the follow-up period of up to 3 months after the initial surgery). The purpose of the study. To assess the feasibility of using intraoperative angiographic imaging during open arterial reconstructions in the infrainguinal segment.Materials and methods. The study is based on the results of an analysis of 80 patients with atherosclerosis of the lower limb arteries who underwent open arterial reconstructions in the infrainguinal segment at the Department of Vascular Surgery at the N.I. Pirogov City Clinical Hospital No. 1 in 2025.Research results and discussion. In the presented study, the incidence of early thrombosis after infrainguinal reconstructions without the use of intraoperative imaging was 1,75 %. If follow-up angiography had been performed, these complications could have been avoided. In addition, in 1,75 % of cases, residual defects were identified in patients after surgery in the form of hemodynamically significant stenoses of the inflow and outflow arteries.Conclusion. Early prevention and elimination of residual changes identified during follow-up intraoperative angiography help avoid thrombosis following vascular reconstructions and repeat surgical interventions</p></trans-abstract><kwd-group xml:lang="ru"><kwd>инфраингвинальные реконструкции</kwd><kwd>инфраингвинальный сегмент</kwd><kwd>ультразвуковое ангиосканирование</kwd><kwd>интраоперационная ангиография</kwd></kwd-group><kwd-group xml:lang="en"><kwd>infra-lingual reconstructions</kwd><kwd>infra-lingual segment</kwd><kwd>ultrasound angioscanning</kwd><kwd>intraoperative angiographic</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Conte M.S., Bandyk D.F., Clowes A.W., Moneta G.L., Seely L., Lorenz T.J., et al.
