Выполнение реваскуляризации при нейро-ишемической форме синдрома диабетической стопы обусловливает трудности выполнения реконструкции сосудов и тяжелое течение послеоперационного периода, что связано с тяжелыми сопутствующими заболеваниями у этой категории больны. Поэтому рентгенэндоваскулярное лечение синдрома диабетической стопы быстро становится терапией выбора у таких пациентов, в то время как ишемия нижних конечностей, определяет их смертность.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Проанализировать результаты реваскуляризирующих вмешательств, при нейро-ишемической форме синдрома диабетической стопы и определить возможности улучшения этих результатов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проспективное исследование включает 93 больных с язвенно-некротическими поражениями стоп и голени на фоне критической ишемии при нейро-ишемической форме синдрома диабетической стопы. Длительность сахарного диабета у больных составила от 1 месяца (диагностирован при обследовании по поводу язвенно-некротического поражения стоп) до 41 года, медиана – 12 лет (25-й процентиль равен 8 годам, 75-й процентиль – 20 годам).</p><p>Результаты исследования и обсуждение. Успешное лечение больных с язвенно-некротическим поражением стопы при нейро-ишемической форме синдрома диабетической стопы возможно в центрах, где имеются условия для выполнения ангиографического исследования, эндоваскулярных вмешательств, сосудистых ангиореконструкций. Многопрофильный коллектив с широким спектром возможностей реваскуляризации конечности и лечения ран является основным фактором успешного лечения больных с нейро-ишемической формой синдрома диабетической стопы.</p></sec><sec><title>Заключение</title><p>Заключение. Разработка методов ранней диагностики (до язвенно-некротического поражения стоп) и своевременная реваскуляризация являются перспективными направлениями улучшения оказания помощи больным с нейро-ишемической формой синдрома диабетической стоп.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Revascularization for the neuroischemic form of diabetic foot syndrome is associated with difficulties in vascular reconstruction and a difficult postoperative course, due to the severe comorbidities in this patient population. Therefore, endovascular treatment of diabetic foot syndrome is rapidly becoming the treatment of choice for these patients, as lower extremity ischemia is a significant determinant of their mortality.</p></sec><sec><title>The purpose of the study</title><p>The purpose of the study. To analyze the results of revascularization interventions in the neuro-ischemic form of diabetic foot syndrome and identify ways to improve these results.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The prospective study included 93 patients with ulcerative necrotic lesions of the feet and lower leg on the background of critical ischemia in the neuro-ischemic form of diabetic foot syndrome. The duration of diabetes mellitus in patients ranged from 1 month (diagnosed during examination for ulcerative necrotic lesions of the feet) to 41 years, the median was 12 years (the 25th percentile is 8 years, the 75th percentile is 20 years).</p><p>Research results and discussion. Successful treatment of patients with ulcerative necrotic foot lesions in the neuro-ischemic form of diabetic foot syndrome is possible in centers where conditions are available for angiographic examination, endovascular interventions, and vascular angioreconstructions. A multidisciplinary team with a wide range of possibilities for limb revascularization and wound treatment is the main factor in the successful treatment of patients with the neuro-ischemic form of diabetic foot syndrome.</p></sec><sec><title>Conclusion</title><p>Conclusion.
