Варикозная болезнь нижних конечностей является наиболее распространённой формой хронической венозной недостаточности и сохраняет высокую медико-социальную значимость. Эндовенозные методы устранения патологического венозного рефлюкса (ЭВЛО, РЧА, МОСА и др.) в настоящее время рассматриваются как стандарт хирургического лечения. Однако в раннем послеоперационном периоде у части пациентов сохраняются нежелательные веноспецифические симптомы, что связывают с термическим или механохимическим повреждением эндотелия и активацией воспалительных механизмов. Несмотря на широкое применение веноактивных препаратов, их роль в периоперационном сопровождении эндовенозных вмешательств и влияние на эндотелиальную дисфункцию остаются предметом обсуждения, что определяет актуальность настоящей поисково-аналитической работы.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Анализ современных данных о применении флебоактивных препаратов в периоперационном периоде эндовенозного лечения варикозной болезни с акцентом на их влияние на маркеры эндотелиальной дисфункции, воспаление, выраженность клинических симптомов.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Доступная зарубежная и отечественная литература, научные базы PubMed, Google Scholar, Scopus.</p></sec><sec><title>Результаты</title><p>Результаты. Обобщение данных показало, что применение веноактивных препаратов в периоперационном периоде способствует более быстрому регрессу веноспецифических симптомов, уменьшению частоты ранних нежелательных явлений и улучшению показателей качества жизни, также продемонстрировано положительное влияние на маркеры эндотелиальной дисфункции и микроциркуляции, что может рассматриваться как патогенетически обоснованный эффект.</p></sec><sec><title>Заключение</title><p>Заключение. Систематизированы современные представления о механизмах и клинической эффективности венотонической терапии в контексте эндовенозных вмешательств. Обобщенные данные формируют комплексное представление о значении периоперационной фармакологической поддержки.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Varicose veins of the lower extremities are the most common form of chronic venous insufficiency and remain a significant medical and social problem. Endovenous methods for eliminating pathological venous reflux (EVLA, RFA, MOCA, etc.) are currently considered the standard of surgical treatment. However, in the early postoperative period some patients continue to experience undesirable venous-specific symptoms, which are associated with thermal or mechanochemical endothelial injury and activation of inflammatory mechanisms. Despite the widespread use of venoactive drugs, their role in perioperative management during endovenous interventions and their impact on endothelial dysfunction remain a matter of discussion, which determines the relevance of the present analytical review.</p></sec><sec><title>Aim of the study</title><p>Aim of the study. Analysing current data on the use of phleboactive drugs in the perioperative period of endovenous treatment of varicose veins, with a focus on their effect on markers of endothelial dysfunction, inflammation, and the severity of clinical symptoms.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Available international and Russian literature analysed using the scientific databases PubMed, Google Scholar, and Scopus.</p></sec><sec><title>Results</title><p>Results. The analysis has demonstrated that the use of venoactive drugs in the perioperative period contributes to a faster regression of venous-specific symptoms, a reduction in the incidence of early adverse events, and an improvement in quality-of-life indicators. A positive effect on markers of endothelial dysfunction and microcirculation was also observed, which may be considered a pathogenetically justified effect.</p></sec><sec><title>Conclusion</title><p>Conclusion. Current concepts regarding the mechanisms and clinical effectiveness of venotonic therapy in the context of endovenous interventions have been systematised.
