В соответствии с Российскими национальными клиническими рекомендациями по диагностике и лечению заболеваний артерий нижних конечностей, выполнение УЗАС рекомендовано для регулярного мониторинга зоны сосудистой реконструкции в послеоперационном периоде, с частотой проведения каждые шесть месяцев.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить ближайшие результаты артериальных реконструкций инфраингвинального сегмента с помощью УЗАС.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Работа основана на результатах обследования 203 пациентов с атеросклерозом артерий нижних конечностей, перенесших открытые и гибридные артериальные реконструкции на инфраингвинальном сегменте в отделении сосудистой хирургии ГКБ № 1 им Н.И. Пирогова в 2025 году. Всем пациентам (n=203, 100 %) на следующий день после операции выполнялось контрольное УЗАС артерий нижних конечностей и зоны сосудистой реконструкции. Экспертное УЗАС с определением скоростных показателей в зоне сосудистой реконструкции, проксимальнее и дистальнее ее, было выполнено в 167 случаях (82,3 %).</p><p>Результаты исследования и обсуждение. В 1-е сутки п/о периода зона артериальной реконструкции была полностью проходима у 198 пациентов (97,5 %). В 5 случаях (2,5 %) после БПШ, в т.ч. в сочетании с БАП артерий голеней, был диагностирован тромбоз шунта. Всем пациентам выполнена тромбэктомия из бедренно-подколенного шунта, дополненная: БАП артерий голени (в 2-х случаях); БАП со стентированием НПА (в 1 случае); БАП со стентированием дистального анастомоза БПШ и ПкА (в 1 случае); ЭАЭ из ОБА и ГАБ (в 1 случае). Резидуальные гемодинамически значимые стенозы в подвздошном сегменте были выявлены у 6 пациентов (2,9 %). При этом, соотношение внутристенотической пиковой систолической скорости кровотока к престенотической у этих пациентов варьировало в пределах от 3,3 до 3,9. Всем пациентам, с целью профилактики раннего тромбоза сосудистой реконструкции, было выполнено стентирование подвздошного сегмента. Резидуальные гемодинамически незначимые стенозы как на подвздошном, так и на инфраингвинальном артериальном сегментах, были выявлены у 12 пациентов (5,9 %). При этом, соотношение внутристенотической пиковой систолической скорости кровотока к престенотической у этих пациентов варьировало в пределах от 2,1 до 2,9. Всем пациентам, с целью профилактики раннего тромбоза сосудистой реконструкции, была назначена двойная дезагрегантная терапия (Аспирин + Клопидогрел), либо монотерапия (Аспирин) в сочетании с Ривароксабаном 2,5 мг х 2 р/сутки.</p></sec><sec><title>Заключение</title><p>Заключение. Всем пациентам, для оценки выполненной артериальной реконструкции на инфраингвинальном сегменте, выявления резидуальных изменений и остаточных дефектов в артериальном русле с целью последующей профилактики послеоперационных тромбозов, в ранние сроки послеоперационного периода рекомендовано выполнение экспертного УЗАС с определением скоростных показателей в зоне сосудистой реконструкции, проксимальнее и дистальнее ее, оценкой морфологического субстрата поражения артериального русла, соотношения пиковой систолической скорости кровотока (внутристенотической к престенотической).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In accordance with the Russian National Clinical Guidelines for the diagnosis and treatment of diseases of the arteries of the lower extremities, ultrasound is recommended for regular monitoring of the vascular reconstruction area in the postoperative period, with a frequency of every six months.</p></sec><sec><title>The purpose of the study</title><p>The purpose of the study. To evaluate the immediate results of arterial reconstructions of the infrainguinal segment using ultrasound.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The work is based on the results of an examination of 203 patients with atherosclerosis of the arteries of the lower extremities who underwent open and hybrid arterial reconstructions on the infrainguinal segment in the Department of Vascular Surgery of the N.I. Pirogov State Clinical Hospital No. 1 in 2025. All patients (n=203, 100 %) underwent a control ultrasound scan of the arteries of the lower extremities and the vascular reconstruction zone on the day after surgery. An expert ultrasound examination with the determination of velocity parameters in the area of vascular reconstruction, proximal and distal to it, was performed in 167 cases (82,3 %).</p><p>Research results and discussion. On the 1st day of the subcutaneous period, the arterial reconstruction zone was completely passable in 198 patients (97,5 %). Shunt thrombosis was diagnosed in 5 cases (2,5 %) after FPB, including in combination with BAP of the arteries of the legs. All patients underwent thrombectomy from the femoral-popliteal shunt, supplemented by: – Reconstruction of the arteries of the tibia (in 2 cases); – BAP with EIA stenting (in 1 case); – BAP with stenting of the distal anastomosis of FPB and PA (in 1 case); – EAE from CFA and DFA (in 1 case).</p><p>Residual hemodynamically significant stenoses in the iliac segment were detected in 6 patients (2,9 %). At the same time, the ratio of intravenous peak systolic blood flow velocity to prestenotic in these patients ranged from 3,3 to 3,9. All patients underwent iliac segment stenting in order to prevent early vascular thrombosis. Residual hemodynamically insignificant stenoses on both the iliac and infra-lingual arterial segments were detected in 12 patients (5,9 %). At the same time, the ratio of intravenous peak systolic blood flow velocity to prestenotic in these patients ranged from 2,1 to 2,9. In order to prevent early thrombosis of vascular reconstruction, all patients were prescribed dual antiplatelet therapy (Aspirin + Clopidogrel) or monotherapy (Aspirin) in combination with Rivaroxaban 2,5 mg x 2 r/ day.</p></sec><sec><title>Conclusion</title><p>Conclusion.
