Меланома кожи остается серьезной современной проблемой онкологии в России и в многих странах мира.  Согласно данным глобального канцер-регистра за 2022 год было зафиксировано 331,722 новых случаев меланомы кожи, из которых 3,9 % относится к пациентам из Российской Федерации. Отмечается ежегодный прирост заболеваемости меланомой кожи и она ответственна за 80 % смертей в группе злокачественных опухолей кожи.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Провести сравнительную оценку эффективности хирургического стандартного и ультра N-стадирования после биопсии сигнальных лимфоузлов флуоресцентным методом с ICG у пациентов с первичной меланомой кожи туловища и конечностей. Материалы и методы исследования. В одноцентровое проспективное исследование были включены 62 пациента с клинически локализованными стадиями меланомы кожи туловища и конечностей (cT1-4N0M0, ECOG 0-1), которым было выполнено широкое иссечение опухоли кожи с биопсией сигнальных лимфатических узлов (БСЛУ) с применением флуоресцентного ICG-картирования. Для исключения локо-регионарных рецидивов и других видов прогрессирования после радикальной операции пациенты проходили контрольные обследования каждые 3–6 месяцев в зависимости от стадии заболевания.</p></sec><sec><title>Результаты</title><p>Результаты. По результатам планового патоморфологического исследования лимфатические узлы были выявлены у всех 62 пациентов (100 %). Данный метод продемонстрировал высокую диагностическую эффективность. Гистологическое в сочетании с иммуногистохимическим (ИГХ) исследованием выявило наличие метастазов в сигнальных лимфоузлах метастазов у 9 (14,5 %) пациентов (рN1), а их отсутствие у 53 (85,5 %) пациентов (рN0).</p><p>Следует отметить, что при контрольных обследованиях у пациентов с рN0   «БСЛУ-» признаков локо-регионарного рецидива выявлено не было. При контрольных обследованиях через 6 месяцев у 2 пациентов (3,2 %) были выявлены локо-региональные рецидивы, у 1 пациента (1,6 %) выявлены отдаленные метастазы через 18 месяцев после операции, и все эти пациенты были с рN1 «БСЛУ+».</p><p>У 53 пациентов (85,5 %) с гистологически негативными результатами было выполнено расширенное патоморфологическое исследование (ультрастадирование), которое выявило дополнительные микрометастазы лишь у 2 (3,2 %) пациентов и не обнаружив корреляции с рецидивами в периоде наблюдения.</p></sec><sec><title>Заключение</title><p>Заключение. Таким образом, при клинически локализованных стадиях меланомы кожи туловища и конечностей БСЛУ с флуоресцентным картированием ICG представляет собой высокоэффективный диагностический метод, который даёт возможность своевременно и точно проводить N-стадирование меланомы кожи с подбором персонализированной тактики лечения и частоты дальнейшего наблюдения у онколога. Такой подход обеспечивает высокую прецизионность хирургического вмешательства и минимизацию его объема на регионарных лимфоколлекторах.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Cutaneous melanoma remains a significant problem in modern oncology in Russia and many countries worldwide. According to the global cancer registry data for 2022, 331,722 new cases of skin melanoma were recorded, of which 3,9 % were patients from the Russian Federation.</p></sec><sec><title>Objective</title><p>Objective. To evaluate the diagnostic value and safety of sentinel lymph node biopsy (SLNB) using indocyanine green (ICG) fluorescence mapping in patients with clinically localized stages of trunk and extremity melanoma.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A single-center prospective study included 62 patients with clinically localized stages of trunk and extremity melanoma (cT1-4N0M0, ECOG 0-1) who underwent wide excision of the skin tumor with sentinel lymph node biopsy (SLNB) using ICG fluorescence mapping. To exclude loco-regional recurrence and other types of progression after radical surgery, patients underwent follow-up examinations every 3–6 months depending on the disease stage.</p></sec><sec><title>Results</title><p>Results. Lymph nodes were identified in all 62 patients (100 %). This method demonstrated high diagnostic value: sensitivity – 100% and specificity – 100 %. Immunohistochemical (IHC) examination revealed the presence of metastases in sentinel lymph nodes in 9 (14,5 %) patients, and their absence in 53 (85,5 %) patients (pN0).</p><p>Notably, follow-up examinations of patients with no metastatic lymph node involvement pN0 ("SLNB-") showed no signs of loco-regional recurrence. During follow-up examinations at 6 months, loco-regional recurrences were detected in 2 (3,2 %) patients, and distant metastases were detected in 1 (1,6 %) patient 18 months after surgery; all these patients had pN1 ("SLNB+").</p><p>In a subset of patients with histologically negative results, extended pathological examination (ultrastaging) was performed, which revealed additional micrometastases in only 2 (3,2 %) patients. This finding did not change the clinical stage and showed no correlation with recurrences during the follow-up period.</p></sec><sec><title>Conclusion</title><p>Conclusion. Thus, in clinically localized stages of trunk and extremity melanoma, SLNB with ICG fluorescence mapping is not only safe but also represents a highly effective diagnostic method that enables timely and accurate staging of cutaneous melanoma with personalized treatment selection and further oncological follow-up.
