Для выполнения пластических операций, в частности, абдоминопластики характерен высокий риск осложнений, в первую очередь серомы. Предполагается, что причиной плохих результатов заживления ран у этих пациентов могут быть протеолитические нарушения. При этом считают, что в качестве факторов прогноза риска осложнений могут быть рассмотрены некоторые биологические маркеры. Целью работы был анализ данных литературы о роли биологически активных веществ как факторов риска развития осложнений абдоминопластики. Результаты. В ряде исследований продемонстрировано различной влияние операций по контурной пластике тела на метаболизм глюкозы. При этом отмечено значительное улучшение чувствительности к инсулину после вмешательств, однако не выявлено значительной нормализации уровня глюкозы натощак. Сведения об изменениях липидного профиля пациентов, которым выполняются пластические вмешательства, противоречивы, в ряде работ отмечена значительная нормализация уровней липопротеидов высокой плотности. В свете современных представлений о роли жировой ткани как источнике провоспалительных цитокинов (в частности, интерлейкина-4) очевидно, что липосакция при выполнении абдоминопластики может способствовать снижению выраженности воспалительных реакций в организме. Обращено внимание на возможность использование С-реактивного белка в качестве маркера прогноза осложнений при выполнении контурной пластики тела. Отмечены перспективы оценки активности ферментов семейства матриксных металлопротеиназ, роль которых как провоспалительных медиаторов и модуляторов иммунитета в настоящее время является общепризнанной. Заключение. Сделано заключение о необходимости дальнейшего совершенствования инструментов прогноза развития осложнений при выполнении пластических операций, в частности абдомино- и контурной пластики</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Performing plastic surgery, in particular, abdominoplasty, is characterized by a high risk of complications, primarily seromas. It is assumed that the cause of poor wound healing results in these patients may be proteolytic disorders. At the same time, it is believed that some biological markers can be considered as factors for predicting the risk of complications. The aim of the work was to analyze the literature data on the role of biologically active substances as risk factors for the development of complications of abdominoplasty. Results. A number of studies have demonstrated the different effects of body contouring surgery on glucose metabolism. At the same time, there was a significant improvement in insulin sensitivity after the interventions, but no significant normalization of fasting glucose levels was detected. Information about changes in the lipid profile of patients undergoing plastic surgery is contradictory, and a number of studies have noted a significant normalization of high-density lipoprotein levels. In the light of modern ideas about the role of adipose tissue as a source of pro-inflammatory cytokines (in particular, interleukin-4), it is obvious that liposuction during abdominoplasty can help reduce the severity of inflammatory reactions in the body. Attention is drawn to the possibility of using C-reactive protein as a marker for predicting complications during body contouring. The prospects of evaluating the activity of enzymes of the matrix metalloproteinase family, whose role as pro-inflammatory mediators and modulators of immunity is currently widely recognized, are noted. Conclusion.
