Трансмуральные дефекты верхних отделов желудочно-кишечного тракта (ЖКТ) остаются одной из наиболее сложных клинических проблем, ассоциированных с высоким риском осложнений и летальности. Разработка малоинвазивных методов, в частности эндоскопической вакуумной терапии (ЭВТ), открывает новые возможности в их лечении. Цель исследования. Оценить эффективность и безопасность однокомпонентной методики ЭВТ у пациентов с трансмуральными дефектами верхних отделов ЖКТ. Материалы и методы. Проведено ретроспективное исследование, включившее 65 пациентов с трансмуральными дефектами верхних отделов ЖКТ различной этиологии за период с января 2021 года по октябрь 2023 года в клинике ММНКЦ им. С.П. Боткина. Всем пациентам проводилось лечение методом ЭВТ в режиме монотерапии с индивидуальным подбором уровня отрицательного давления и регулярной заменой макропористой губки. Эффективность оценивалась по количеству сеансов, длительности госпитализации, частоте закрытия дефектов и наличию осложнений. Результаты. Закрытие дефекта достигнуто у 86,2 % пациентов. Наименьшее количество сеансов потребовалось при острых перфорациях (в среднем 7,6), в то время как при несостоятельности швов анастомоза и хронических свищах лечение было более длительным. Внутриполостная установка системы позволила сократить число замен и сроки госпитализации. Нежелательные явления отмечались эпизодически и купировались консервативно. Летальность составила 12,3 %, в основном вследствие тяжёлого соматического статуса пациентов, а не прогрессирования локального процесса. Заключение. ЭВТ в режиме монотерапии является эффективным и безопасным методом лечения трансмуральных дефектов верхних отделов ЖКТ и может рассматриваться как метод первой линии у пациентов с противопоказаниями к хирургическим вмешательствам.</p></abstract><trans-abstract xml:lang="en"><p>Background. Transmural defects of the upper gastrointestinal (GI) tract remain a challenging clinical condition associated with high morbidity and mortality. Minimally invasive techniques, particularly endoscopic vacuum therapy (EVT), have emerged as a promising treatment option. Objective. To evaluate the efficacy and safety of EVT applied as monotherapy in patients with transmural defects of the upper GI tract. Materials and methods. A retrospective study included 65 patients treated with EVT at a single tertiary center (2021–2023). Negative pressure was individually adjusted (−70 to −120 mmHg), with sponge replacement every 3–5 days. Outcomes were evaluated by defect closure rate, number of sessions, hospital stay, and complications. Results. Defect closure was achieved in 86,2 % of cases. Acute perforations required fewer sessions, while anastomotic leakage and chronic fistulas demanded prolonged treatment. Adverse events were rare and manageable; mortality (12,3 %) was mainly due to comorbidities. Conclusion.
