В условиях ограниченной эффективности консервативных методов терапии бариатрическая хирургия рассматривается как наиболее действенный способ достижения устойчивого снижения массы тела и улучшения метаболических показателей у пациентов с морбидным ожирением. В настоящее время не существует четкого алгоритма выбора первичной бариатрической операции, что значимо затрудняет работу начинающих бариатрических хирургов и мультидисциплинарной команды. Большинство специалистов основываются на мнении ведущих бариатрических хирургов и клинических рекомендациях по лечению ожирения.</p><p>Материал и методы исследования. Проведен анализ современных подходов к выбору оптимального вида бариатрической операции на основании клинических рекомендаций ведущих международных и российских организаций (IFSO, ASMBS, NICE, РОХ, ОБХ), данных актуальных исследований.</p></sec><sec><title>Результаты</title><p>Результаты. На основании перечисленных преимуществ, недостатков и категорий пациентов сформирован алгоритм выбора первичной бариатрической операции для пациентов с ожирением и метаболическим синдромом.</p></sec><sec><title>Заключение</title><p>Заключение. Разработанный алгоритм демонстрирует, что выбор первичной бариатрической операции основывается на множестве факторов, влияющих на безопасность и эффективность оперативного вмешательства. Применение данного алгоритма позволяет систематизировать процесс принятия решения и адаптировать хирургическую тактику под индивидуальные особенности пациента, что способствует снижению риска осложнений и улучшению результата.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Given the limited efficacy of conservative treatment methods, bariatric surgery is now considered the most effective approach for achieving sustained weight loss and metabolic improvement in patients with morbid obesity. Currently, there is no clearly defined algorithm for selecting primary bariatric procedures, which significantly complicates decision-making for early-career bariatric surgeons and multidisciplinary teams. Most specialists rely on expert opinions from leading bariatric surgeons and clinical obesity treatment guidelines.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. We analyzed contemporary approaches to optimal bariatric procedure selection based on clinical guidelines from major international and Russian organizations and current research data.</p></sec><sec><title>Results</title><p>Results. Based on the comparative benefits, limitations, and patient-specific factors, we developed an algorithm for selecting primary bariatric procedures for patients with obesity and metabolic syndrome.</p></sec><sec><title>Conclusion</title><p>Conclusion. The proposed algorithm demonstrates that selection of primary bariatric surgery should be based on multiple factors affecting procedural safety and efficacy.
